For many years, scientists thought the thymus was only important for childhood immunity. It shrinks after puberty and was believed to have little impact on adult health.
However, new research shows this overlooked organ might actually predict how long we live, how well we fight off illness, and even how successful cancer treatments can be.
Researchers at Mass General Brigham used artificial intelligence to study CT scans from tens of thousands of adults.
Their results, published in Nature, found that people with healthier thymuses lived longer and had lower risks of heart disease, cancer, and death. These findings challenge long-held beliefs and open new avenues for understanding ageing and the immune system.
“The thymus has been overlooked for decades and may be a missing piece in explaining why people age differently, and why cancer treatments fail in some patients,” said Hugo Aerts, PhD, director of the Artificial Intelligence in Medicine Program at Mass General Brigham.
Understanding the Thymus: Its Role and Importance
The thymus sits in the chest and helps train T cells, which are immune cells that protect us from infections and disease. Since the thymus shrinks after puberty, scientists used to think it was not important in adults.
Earlier studies suggested a link between T cell diversity and ageing, but they were small. This new research analysed data from over 25,000 adults in a national lung cancer screening trial and 2,500 participants in the Framingham Heart Study. The researchers used AI to measure the thymus’s size and structure, creating a ‘thymic health’ score.
How AI Uncovered the Thymus–Longevity Connection
The results stood out. Adults with healthier thymuses had about a 50% lower risk of dying from any cause, a 63% lower risk of dying from heart disease, and a 36% lower risk of getting lung cancer.
These links remained strong even after accounting for age and other health factors.
The study also found that when thymic health declines, T cell diversity drops, making it harder for the immune system to fight new threats.
Chronic inflammation, smoking, and being overweight were all connected to poorer thymic health, which suggests that daily habits might matter.
The Thymus’s Impact on Cancer Therapies
In another study of over 1,200 cancer patients who received immunotherapy, those with healthier thymuses had better results.
They had a 37% lower risk of their cancer getting worse and a 44% lower risk of dying compared to patients with less healthy thymuses.
Overall, these findings highlight that the thymus may play a bigger role than previously thought in how well modern cancer immunotherapies work.
The researchers say that more studies are needed. Right now, the imaging method for assessing thymic health is not ready for routine use in clinics, and it is still unknown whether lifestyle changes can directly improve thymus function.
Ongoing research is examining whether radiation to the thymus during cancer treatment affects outcomes.
In the future, checking thymic health might help doctors assess disease risk, choose treatments, and find new ways to protect the immune system as we get older.
Rediscovering the Thymus: From Neglect to Newfound Importance
Once thought to be unimportant, the thymus might actually be a hidden key to living longer and staying healthy.
Its condition could help explain why some people age well while others get sick, and why some cancer patients respond to immunotherapy while others do not.
This once-forgotten organ may now be at the center of future medical advances.
